The Laser Pain Center Huntsville, LLC hereafter LPCH includes its doctors, providers, employees, agents and assigns.  Dr Greg Millar nor LPCH does not accept nor are we providers for any insurance including but not limited to BCBS, BCBS of AL, Medicare, Medicaid, Tricare, or any other State or Federal insurance at the LPCH. Results vary case to case and NO guarantee or warranty of outcome is made.  Nothing works for everyone or everything. The Free/No Cost Consultation is just that, a meeting to discuss your problems and our solutions. Additional fees or cost will be charged in individual cases for additional treatment or visits should you become a patient. Charges vary based on services rendered. Content provided as described above including but not limited to answers to specific questions asked by you about your specific medical condition(s), attending talks or Lunch & Learn lectures, Free consultation, screening you individually, touching, palpating or even examining your spine or body DOES NOT create a doctor-patient relationship between you and any of the doctors employed by or affiliated with LPCH.  A doctor-patient relationship with one of the LPCH doctors is ONLY achieved by a "prospective patient" filling out and signing appropriate LPCH paperwork and forms including but not limited to intake forms, Hipaa forms, informed consent forms, and other forms as required by LPCH and the individual doctor and then that "prospective patient" meeting with the LPCH doctor face to face at the office of the LPCH and that "prospective patient" being accepted as a "patient" by that individual LPCH doctor.  The acceptance of which must be in writing in the chart as well as verbally to the "prospective patient" by the LPCH doctor. The LPCH doctors appear on live TV. They are doctors and not TV professionals. They give their "best efforts" and try hard to provide accurate and truthful information, Content without errors or omissions.  Live TV is inherently fraught with mistakes and mis-statements.  Live TV has three basic problems. First is the time constraint of live TV. With live TV, there is not enough time to go into details and specifics about Content.  Secondly, with live TV the LPCH doctors are asked by the station to speak in "people speak" instead of technical chiropractic or medical terms that the average viewer or listener may not understand.  The LPCH doctors give their "best effort" to accurately report and discuss all topics and render information and opinions as Content.  Third, live TV waits for no man, meaning the doctor is often put on the spot having to come up with something to say. Again, the LPCH doctors use their best efforts to provide accurate Content.  In giving their "best effort" and speaking in people speak, and being put on the spot, often mistakes are made including but not limited to mis-statements, errors, omissions, malapropism, slip of the tongue mis-information, hereafter know as Mal-Content.  We apology in advance for those mistakes. LPCH nor it's doctors individually or collectively shall be held liable for any Mal-Content including but not limited to mis-statements, errors, omissions, malapropism, slip of the tongue, inaccuracies or delays or for any actions taken in consequence as we make no warranties implied or stated as to the correctness of content.  All persons receiving any Content or viewing or listening to broadcast of Content, or talking to a LPCH doctor in person receiving Content, are duty bound by the following "Terms Of Use":  Should you have issue with Content then You, as Claimant, must first notify Dr Greg Millar, DC., by certified mail at PO BOX 21157 Huntsville AL 35813 of any mis-statements in Content or Mal-Content. Your act of making a claim of mis-statement, omission or Mal-content constitutes an agreement by and between you the Claimant and LPCH its doctors, providers and employees.  Then, Dr Millar has 20 business days to respond to you in writing by certified mail at the address stated on your complaint of our response to your claim of mis-statement, omissions or Mal-Content.  Should we agree that the mis-statement, omission or mal-Content needs correcting, then our remedy is within 30 days of our written response to you for the LPCH doctor to retract and correct the mis-statement, omission or Mal-Content either on the air, OR on our website.  Should the mis-statement of Content, Omission or Mal-Content be retracted and restated correctly or the corrected Content statement be published on our website for 30 days, then NO other remedies may be sought after and your retraction by LPCH or publishing corrected Content on our website shall constitute good consideration for the General Release as stated below by and between you and LPCH its doctors, providers and employees.  

All videos on this site are used for informational purposes only. Videos on this site contain persons that are not LPCH patients. Some of these videos are marketing videos provided by the manufacture of the MLS Laer, Anadyne Therapy, ReBuilder Medical and other devices used at LPCH. Some videos (those with Dr Millar or patients interviewed by Dr. Millar) are patients of LPCH and are used with permission of the patient.

Patient prepayment monies over $500 collected by LPCH for future services are placed in a separate LPCH escrow account. After treatment, the monies are taken out of escrow. Patient payments for ReBuilders Medical units and Supplies are not placed in escrow. Patient payments for that date of service are also not placed in escrow. Should a person prepay and not use the services then they are due a refund of the unused portion. To receive a refund, the patient needs to fill and sign the LPCH written refund request form. Refunds are processed within 10 business days of written request.

Several places in our Content, we use the following:  FREE* - Should the person receiving Free* services as outlined above be accepted as a patient of LPC then additional charges for treatment will be incurred in individual cases as needed for related services.  Again some persons are not allowed to receive Free services, If that is your case then this does not apply to you.

Terms of use: For good consideration, in the form of a retraction or restatement of Content, or from LPCH publishing corrected Content on its website for 30 days, the receipt of good consideration which is hereby acknowledged, the claimant party, Releaser, does hereby forever remise, release, aquit, satisfy, and forever discharge LPCH its employes, providers and doctors, of and from all manner of actions, causes of actions, proceedings at law or administrative law action or equity actions, suits, debts, covenants, contracts, controversies, agreements, promises, claims and demands whatsoever, which said claimant ever had, now has, or which any personal representative, successor, heir or assign of said releaser Claimant, hereafter can, shall or may have, against LPCH, by reason of any matter, cause of thing whatsoever related to the Claimants' assertion of mis-statement, Mal-Content or Omissions, from the beginning of time. Claimant hereby agrees to indemnify LPCH and pay LPCH's cost of defense of said General Release to and include reasonable attorneys fees. This General release shall be construed and the validity, performance and enforcement hereof shall by governed by the Laws of the State of Alabama.
